She also performed root canal therapy on tooth number 30 without charting rubber dam usage, crown cementation, the material used to temporize the tooth, working length and number of canals, the type of sealer and irrigant used, and the build-up and desensitizer that the patient was charged for. The dentist also utilized incorrect CDT billing codes (e.g.. D2999 for Optional Treatment Fee; D9911 for Desensitizer; D2999 for Porcelain Butt Margin; and D2999 for Custom Temp Crown) for treatment provided on non-vital tooth number 30 on April 23, 2014.
The dentist also performed a composite restoration on tooth number 13 without adequate documentation of the etch used, the bonding agent used, and the desensitizer that the patient was charged for. In addition, she charged the patient for desensitizer for the composite restoration of tooth number 13, when desensitizer is already part of the composite restoration code she charged the patient for. Further, the use of desensitizer was unnecessary on tooth number 13 because it was a shallow preparation, as evidenced by the non-usage of anesthetic during preparation.
The Board took action to the case, summoning the dentist to defend against the accusation. Unfortunately, the dentist failed to hire a good dental attorney to help her defend her case.
As a result, the dentist was sentenced to disciplinary action to ensure that she will never commit such acts again.
