Specifically, her records failed to adequately document vital signs on May 1, 2014, the office at which the doctor worked at the time failed to retain his record of the treatment provided on June 26, 2014, and an explanation as to why the items above were not recorded.
This is the reason why the dentist’s license was disciplined and suspended.
