During the time period from April 16, 2014, to December 10, 2014, the dentist fell below the minimum standard of care of a patient in the placement of implants and in bone grafting. Specifically, she failed to monitor the patient’s vital signs during a surgical procedure and failed to thoroughly clean the failing implant site at tooth number 6 with debridement before placing bone graft; and iii. placed a new implant of the same size at tooth number 4 one month after discovering the implant was failing and two months after the initial placement of the implant.

During the time period from October 15, 2014, through December 10, 2014, the dentist failed to make, maintain, and keep adequate dental records for a patient. Specifically, she failed to record baseline vital signs, vital signs before administering anesthesia, amount and type of anesthetic used, whether healing caps or healing screws were used during placement of implants whether a second surgery would be required to expose the dental implants and an explanation why these items were not included.

Unfortunately, because of lack of a dental attorney to help her, the dentist has been subjected to disciplinary action.
