Specifically, the records did not include findings and charting of the clinical and radiographic oral examination and diagnostic radiographs, or any statement as to why these items were not recorded.
The dentist was given the chance to defend against her case and present herself to the Board. Despite her explanation, the Board placed her under disciplinary action but without any suspension whatsoever.
