Specifically, the record does not include vital signs, an explanation why vital signs were not obtained, findings and charting of clinical and radiographic oral examination including periodontal charting, a treatment plan including recommendations and treatment options and written informed consent signed by the patient.
Because of the patient’s complaint and enough evidence which proved the errors of her actions, the dentist was disciplined. Additionally, she failed to hire a dental attorney for assistance, causing her to lose her license.
