On or about February 29, 2008, the dentist fell below the minimum standard of care in the dental treatment of her patient when she failed to meet the minimum standard of care in root canal therapy on tooth #24 of the patient.

During the same period of time, the record does not include a written informed consent signed by the patient covering all treatment provided; a notation of initial blood pressure and pulse, or a statement explaining why these vital signs were not obtained.

The investigation found that the dentist fell below the minimum standard of practice for dentists. This is the reason why the dentist’s license was disciplined and suspended.
