d. the patient’s level of consciousness, oxygenation, ventilation, and circulation prior to discharge.

During the time period from October 4, 2010, through February 2014, the dentist fell below the minimum standard of care by failing to make, maintain, and keep adequate records for several patients. Specifically, the records did not include:

a. vital signs, including but not limited to blood pressure and heart rate;
b. findings and charting of clinical and radiographic oral examination;
c. diagnoses;
d. a treatment plan including recommendations and treatment options;
e. an adequate record of treatment provided ;
f. medication and dosages are given to the patients;
g. an explanation of why items (a) through (f) were not recorded.

The dentist was given the chance to defend her case. She states that everything was just an accident and nobody wanted it to happen. She further states that she did everything she could to treat the patient with caution.
