During the time period from April 14, 2007, through June 15, 2011, the dentist fell below the minimum standard of care by failing to make, maintain, and keep adequate dental records on several patients when their records do not include vital signs, including but not limited to blood pressure and heart rate, or an explanation why vital signs were not obtained; written informed consent signed by the patient or the patient’s guardian; findings and charting of clinical and radiographic oral examination; a treatment plan, including recommendations and treatment options; or diagnosis.

The Board gave the dentist the chance to defend herself. However, she was not able to provide a good defense for herself, especially when the dental results were brought up to her. Therefore, the Board placed her license to a disciplinary action instead.
