During the time period from July 25, 2011, through September 1, 2011, the dentist fell below the minimum standard of care by failing to make, maintain and keep adequate records for her patient. Specifically, the records did not include:

a. findings and charting of clinical and radiographic oral examination; diagnoses, and treatment performed on August 22, 2011;
b. a treatment plan including recommendations and treatment options; and
C. vital signs, or an explanation of why they were not recorded.

The dentist was given the chance to defend against her case and present herself to the Board. Despite her explanation, the Board placed her under disciplinary action but without any suspension whatsoever.
