e. vital signs taken prior, during, and after the administration of nitrous oxide/oxygen;
f. a treatment plan including recommendations and treatment options;
g. a written informed consent signed by minor patient’s guardian;
h. an appropriate time-oriented anesthetic record for the administration of nitrous oxide/oxygen, including documentation of the individuals present during administration of the nitrous oxide/oxygen and the start and finish times of the inhalation agents; and i. an explanation why items a.-h. were not present.

Therefore, the Board decided to summon the dentist for a hearing to defend against the complaint filed to her – this is the part where a dental attorney should be hired.
