- She failed to perform the necessary diagnostic procedures, such as a diagnostic wax-up, in order to plan for the patient’s lack of clinical crown height, the appropriate envelope of function, and the generalized severe wear of the patient’s anterior teeth;
- She failed to provide the laboratory with necessary information regarding the proposed length of the anterior crowns and the left to right resulting in the laboratory choosing the incisal edge position of the anterior crowns;
- She failed to address the issue of lack of clinical crown height prior to treatment and instead removed tooth structure from the incisal edges of the mandibular teeth in order to make room for the crowns;
- She failed to provide adequate tooth reduction, which was necessary to provide for the appropriate thickness of the restorative material; and
- She failed to follow-up with the patient on the use of a night guard or a bruxism appliance, which was necessary in order for the treatment to succeed.
As a result, several of the crowns dislodged and the patient’s teeth require re-prep and new crowns.
The dentist was given the chance to defend against her case and present herself to the Board. Despite her explanation, the Board placed her under disciplinary action but without any suspension whatsoever.
