Specifically, she failed to use a rubber dam, failed to diagnose the pulpal and periapical status of the tooth, failed to use an irrigation solution, overfilled the canals of the tooth, separated a file in the tooth, failed to refer the patient to a specialist for an evaluation, and placed a crown on the tooth without addressing the overfilled canals or separated file.

Additionally, during the said period of time, the dentist also failed to make, maintain, and keep adequate dental records on a patient. Specifically, the records do not include:

a. vital signs, including but not limited to blood pressure and heart rate;
b. documentation of radiographs taken and findings deduced from them, including radiograph films or digital reproductions;
c. written informed consent signed by the patient;
d. type of anesthetic given, or how it was given;
e type of permanent build-up restoration;
f. type of irrigation solution used; g. adequate documentation regarding the cleaning and shaping of the canals;
h. an explanation of why the required items were not recorded.

Because of this, the patient filed a complaint and the dentist was summoned to hear her side. Unfortunately, with the lack of an effective dental attorney to defend her case, the Board decided to take disciplinary action against the dentist.
