During the time period from September 30, 2010, through November 2, 2010the records of the said patient did not include:
a. findings and charting of clinical and radiographic oral examination;
b. diagnoses necessitating surgical removal of tooth roots and bone grafts for teeth number 15 and 16; and
C. a treatment plan including recommendations and treatment options.
This is the reason why the dentist’s license was disciplined and suspended.
