Specifically, the records do not include a treatment plan including recommendations and treatment options.

The Board gave the dentist the chance to defend herself. However, she was not able to provide a good defense for herself, especially when the dental results were brought up to her. Therefore, the Board placed her license to a disciplinary action instead.
