During the time period from March 30, 2010, through September 29, 2014, the dentist fell below the minimum standard of care and engaged in dishonorable conduct during the dental treatment of a patient. Specifically, she prescribed Hydrocodone, a Schedule II controlled substance, to the patient for a non-dental purpose.

During the said periods of time, the dentist again 4 fell below the minimum standard of care by failing to make, maintain, and keep adequate records for the patient. Specifically, the records did not include; a. medications and dosages were given to the patient; b. diagnoses explaining the necessity for all prescriptions of narcotic pain medications; c. prescription records that meet the requirements of the Texas Department of Public Safety (DPS), Drug Enforcement Agency (DEA). and the Texas Controlled Substances Act, including but not limited to a prescription drug log and a centralized inventory of controlled substances; or d. an explanation why the above items were not recorded.

Unfortunately, because of the lack of a dental attorney to help her, the dentist has been subjected to disciplinary action.
