making record entries, if different from the provider dentist; or

  1. any statement as to why items a. – f. is not recorded.

Because of the patient complains and enough evidence which proved the errors of her actions, the dentist was disciplined. Additionally, she failed to hire a dental attorney for assistance, causing her to lose her license.
