On or about October 30, 2009, she administered 300 mg of Ketamine and 5 mg of Versed in the span of 25 minutes.

Additionally, during the same day October 30, 2009, the dentist released the patient without documenting that she appropriately monitored her patient prior to discharge.

The investigation found that the dentist fell below the minimum standard of practice for dentists. This is the reason why the dentist’s license was disciplined and suspended.
